Absence Request Form

The Office of the Dean of Students will review absence requests for medical, family, or personal emergencies with appropriate documentation.

For absences such as interviews, regular appointments, weddings, personal travel, or other non-emergency leave please consult your syllabi and communicate directly with your faculty members. Contact your instructors as soon as you become aware of a potential absence via email, office hours, in-person, or their preferred form of communication.

Arizona State University's accommodation for religious practices requires faculty to reasonably accommodate religious practices of students upon request. Students should make accommodation requests directly to their instructors with as much notice as possible.

NOTE: Students should review the course syllabus and attendance policies, as the Office of the Dean of Students does not have authority over instructor attendance or grading policies.
